Hypospadias Setbacks to Surgical Success: A Gurgaon Child’s Journey of Hope

A 10-year-old male child from Gurgaon, Haryana, presented with a history of penoscrotal hypospadias. He had undergone his first hypospadias repair surgery in July 2023 at the age of 9  years in Medanta Hospital Gurgaon. Following catheter removal after the initial surgery, he developed severe pain and burning during urination and was unable to empty his bladder completely. Over the next two months, his urinary stream progressively weakened, and he began experiencing recurrent urinary tract infections.

He was managed with multiple urethral dilatations for the poor urinary stream; however, the procedure was painful and ineffective. As his condition worsened and he became unable to pass urine per urethra, a suprapubic catheter (SPC) was inserted to allow urinary drainage directly from the bladder.

The child and his parents became increasingly distressed and sought further treatment options. They contacted hypospadias foundation and travelled to Navi Mumbai to meet us in our Hypospadias Clinic at MITR Hospital.

On examination at hypospadias foundation clinic, the urethral opening (meatus) was found to be completely stenosed, preventing urination through the meatus. After assessment we explained to the parents that the previous repair has failed and that redo surgeries in 2-3 stages would be required to completely correct it.

The first step involved cystoscopic evaluation. A small-caliber endoscope was introduced to assess the reconstructed urethra, which was found to be completely obliterated. The native urethra with normal calibre was identified at the proximal penile level, and a new urethral opening (urethrostomy) was created at this site to restore urinary flow. Following this, the suprapubic catheter was removed, and the child was able to pass urine normally through the new opening. This gave the child an immediate relief and he became pain free.

After a 4-month interval, Stage 1 repair was performed. This involved excision of the unhealthy, strictured urethral segment and placement of an oral mucosa graft.

Eight months later, stage 2 repair (urethroplasty) was completed by tubularizing the graft and reconstructing a new urethra. The urinary catheter was removed 12 days postoperatively. Aftercatheter removal, the child voided with a good, single, straight urinary stream from the tip of penis without pain or discomfort.

At follow-up more than one year after stage 2 repair, the child remains asymptomatic with no complications. Long-term follow-up is planned, including reassessment at around 15 years of age.

I, Monika, would like to share my experience of my son’s treatment for hypospadias.

My son was diagnosed with hypospadias, and we initially got his surgery done in Gurgaon by a highly reputed surgeon known for treating such cases. His first surgery was performed in April 2023. However, after the surgery, my son was in severe, excruciating pain. He had a urinary catheter for 15 days, and once it was removed, he faced significant difficulty while passing urine.

When we consulted the surgeon, we were told that this was normal. But as parents, we felt something was not right. When his condition did not improve, we went back again, and we were advised that a second surgery would be needed. This was done in May 2023, just one month after the first surgery. Unfortunately, even after the second surgery, he was still unable to pass urine properly.

He then underwent a third surgery in June 2023, but sadly, that too was unsuccessful.

We also sought opinions from some of the top hospitals in Delhi, but we were not fully confident and did not want to take any further risks with our child.

Finally, we decided to travel to Mumbai and consult Dr. Singal and Dr. Shenoy. We met them in August 2023, and from the very first consultation, we felt reassured that we had found the right place for our son. They explained everything with complete transparency and told us that he would need 2–3 surgeries, but that a full recovery was achievable.

My son underwent two surgeries, spaced 8 months apart, and after these procedures, he recovered completely. What stood out the most was that during these surgeries, he did not experience even a fraction of the pain he had endured earlier.
