<p>Hypospadias is a common congenital birth defect which occurs in 1/150 boys in India. 20-30 years back the treatment for hypospadias was very not very successful as the procedure and surgical techniques very not very refined, the number of expert hypospadias surgeons focussed on treatment was less, sutures and instruments for hypospadias surgery were also very basic. With advancing knowledge and expertise, these days in 2019, the results of hypospadias repair (urethroplasty) are much better. A lot of adults in this century, underwent surgery as children way back in 1980’s and 1990’s and some of them did not get good results even after multiple hypospadias surgeries due to complications. Some of these people lost hope at a cure and did not pursue further surgery for failed hypospadias. At Hypospadias Foundation, we regularly get such young adults who come from all over India to seek best treatment and cure for Hypospadias. One of the new techniques to cure such adults with hypospadias is to use skin from inside the mouth for making new urethra- called oral or buccal mucosa graft urethroplasty. In hands of an expert surgeon, it has extremely good results.</p>
<p><strong>Case History:</strong></p>
<p>Mr A.G. 26 years old young man, engineer by profession, extremely smart and suave in nature, came to our hypospadias clinic one evening. He sat down and talked about the trauma of two failed hypospadias surgery which he underwent in Indore with a plastic surgeon. The last surgery was about 10 years back. He had lost hope for cure till he stumbled upon Hypospadias Foundation while searching online. He had come with a hope to get a cure for his hypospadias issue and get married soon. His present problem was thin stream of urine, pain while passing and leak of urine from underside of penis. On examination, his penis was straight (no chordee), he had a fistula in proximal penile region with thin unhealthy urethra. We sat with him and explained the plan to do a cystoscopy and check for quality of urethra and find out reason for repeated failure of surgery and then decide whether a one stage or two stage surgery is needed for full cure and best results of hypospadias surgery. A.G. agreed for the plan and wanted the best treatment.</p>
<p><strong>Surgery:</strong></p>
<p>On cystoscopy, we saw that his new urethra was thin and unhealthy. We cut it open from tip of penis to the site of fistula and actually little below that. We saw that the dorsal wall (roof of urethra) was healthy with minor scarring. We made a deep cut in the dorsal wall in the central midline. We took a buccal graft (oral or labial graft) from upper lip. The graft was fixed with absorbable sutures in the midline to form a part of the neo-urethra. The graft and surrounding urethral wall were rolled into a tube with absorbable sutures around a 14 Fr silicon catheter – all in one stage. The unhealthy skin was excised, and closure of skin also done in a cosmetic way. The catheter was removed after 3 weeks and at one month after catheter removal AG was passing urine in a thick stream without pain from the tip of penis. He was very happy and planning to get married soon. We advised him to marry atleast 6 months after hypospadias surgery. The new urethra is still a bit fragile for 3-4 months after surgery and it is better to wait for 6 months for intercourse (coitus/ sex) after surgery.</p>
<p><strong>Buccal (oral) mucosa graft urethroplasty in adults:</strong></p>
<p>An adult with multiple failed hypospadias surgeries, generally has no normal penile skin or prepuce left for hypospadias repair using local tissues from penis. Hence in such patients, it may be better to use skin from inside the mouth for making the new passage. The skin can be easily taken from inside of cheek or lips. The defect heals very fast and generally without much complications. The buccal graft adapts to penile location very well and has great long-term results in failed surgery. While some of these cases are suitable for a single stage surgery, some may require two stage surgery depending on the severity of the defect. Nonetheless, Buccal Graft Urethroplasty remains a great option with best long-term results for children and adults with Failed Hypospadias.</p>

			<p>Hypospadias is one of the most common urological anomalies in boys. While 80% of all hypospadias are mild and of distal or midpenile, 20% are proximal hypospadias. Severe varieties like scrotal hypospadias form only 5% of total burden of hypospadias. Severe hypospadias such as scrotal hypospadias often have associated chordee (bent penis) and this may be very severe too posing a challenge for the hypospadias surgeon. We have realized in last few years that making the penis straight (chordee correction) is THE MOST IMPORTANT step of hypospadias repair procedure. While most of the severe scrotal hypospadias are also repaired in a single stage (almost 90% are suitable for a single stage urethroplasty), in 10% of the children the anomaly is so severe especially due to severe chordee that two stage repair may be better for an optimal long term outcome.</p>
<p>Master R.P. was born with scrotal hypospadias, severe chordee in Patna, Bihar and was advised to undergo surgery after 1 year of age. The parents went to multiple doctors in Patna, Delhi and finally came to consult Dr A.K.Singal, renowned Hypospadias surgeon at MITR Hospital, Navi Mumbai. On examination, the child had a severe scrotal hypospadias, severe chordee (almost 90 degrees) and abnormally fused skin of scrotum to the penis. This almost looked like a trapped hypospadias penis due to skin shortage. Dr Singal discussed in detail with parents that such a severe hypospadias may require hypospadias treatment in stages. Due to the severity it may be impossible to correct all the defects in one surgery. Also the penis length may have got shortened and cosmetic result would also been suboptimal.</p>
<p>Finally, R.P. was taken under anesthesia and hypospadias repair surgery was started. As a first step the penis was degloved. Urethral plate was divided and the fibrous bands causing chordee on the underside of penis were divided deeply till the scrotum. The penis was almost straight after this maneuver with just 30 degree bend which was further corrected by a placing a placating stitch on the top side of penis (tunica albuginea plication or TAP). Artificial erection test confirmed the complete correction of chordee. Since the chordee was severe and the urethral plate was also divided, single stage surgery would have carried very high failure rates. Hence, a staged urethroplasty was planned. The skin on the top of the penis was divided in midline and brought to the underside of penis. A part of the skin was advanced into the head of penis (glans) so that the new urethra in next stage would be constructed till the tip of penis. This is called Thiersch Byar’s urethroplasty stage-1. There was a compression dressing done which was removed after 7 days and the family travelled back to Patna after a total of 14 days after first hypospadias surgery.</p>

<p>The family sent us some pictures in the healing phase over next few months and the second stage urethroplasty was planned after 6 months of first hypospadias surgery. When we saw the patient again – the flaps and the skin on the underside was very well healed and ready for second stage. Second stage urethroplasty was done and it took just 2 hours for the surgery. The catheter was removed after ten days and the R.P. passed urine in a good stream from the tip of the penis like a normal boy. Family was delighted and after another follow-up visit one week later they travelled back to Bihar.</p>
<p>According to Dr Singal, “With the experience and expertise, currently we do single stage surgery for even scrotal hypospadias and other complex hypospadias. The only limiting factor is the severity of the chordee. Sometimes in sever chordee we need to apply a cut on the underside of penis to straighten it. In such cases, it is better to do a staged repair. In our experience, two stage hypospadias surgery can have excellent cosmetic and functional results. It also provided a longer penis length by straightening the penis and elongating the underside of penis when chordee correction is done.</p>

			<p>Besides 100,000 kids born in India every year with hypospadias, there are an estimated 5-10 lakh adults living with either unoperated hypospadias or failed hypospadias having complications of Hypospadias surgery. These can be minor issues such as urethral fistula, chordee or suboptimal cosmetic results but a significant number may have major issues such as completely failed hypospadias, urethral diverticulum, hairy urethra, urethra stricture etc. Unable to come to terms with penile deformity, they do not get any hope or assurances from adult urologists or plastic surgeons as this may not be the area of primary interest or dedication for them.</p>
<p>Mr Rajveer Singh, 30 years old man, had a midpenile hypospadias with chordee and was operated by a plastic surgeon in Hissar Haryana 2 years back. After surgery, there was a breakdown of hypospadias repair and he was still passing urine from underside of penis. He was still having a downward curvature of penis (chordee) even after this surgery. Also he had seen hair growing out of urinary opening over last few months. He was very worried as he wanted to get married, hence he wrote to us asking for help. We wrote back asking for detailed pictures on email which would help us in understanding his clinical condition and hypospadias deformity. Finally, we called him to see us at Hypospadias Foundation in Kharghar, Navi Mumbai. He took a flight from Delhi to Mumbai and landed up in my Hypospadias clinic on a Monday evening. On examination, he had a urinary opening much below the head of penis in midpenile region and also a couple of holes (urethral fistula) from where he was leaking urine. More importantly his penis still looked bent due to uncorrected chordee. Lastly the cosmetic outcome was really bad with tags of skin here and there and we could see hair coming out of his urethra due to previous failed improper hypospadias surgery. This results when an undertrained hypospadias surgeon uses hairy penile skin for urethral reconstruction. We counselled Rajveer regarding the surgical plan the first step of which would be excision of all unhealthy skin tissue on underside of penis, chordee correction (straightening of penis) and then decide for the type of hypospadias repair in operation theatre. This would be either a single stage repair using a local flap from hairless penile skin or a staged repair using a buccal mucosa graft. In staged buccal mucosa graft – we first excise all unhealthy tissue and scars of failed hypospadias repair, then take a thin lining from inside of lower lip or cheek and then place it as a graft on underside of penis. Once this graft gains local blood supply from penis tissue, we roll it into a urethral tube typically after 6 months of first stage. We always keep buccal graft as the last option in failed hypospadias as that essentially means multiple surgeries. Hence, we attempt to do a single stage flap repair whenever feasible even in failed hypospadias and hence we shared this intent with Rajveer.</p>
<p>Rajveer was taken up for surgery the next day under spinal anesthesia and first a cystoscopy was done. It showed normal urethra beyond midpenile region and unhealthy scarred hairy urethra with fistulae in the distal penile region. We excised all the unhealthy tissues and then checked for chordee. There was still 30 degrees bend in penis which needed correction by a 12 O’clock non-absorbable stitch on the top side of penis. On rechecking with artificial erection, there was no chordee now. After chordee repair was satisfactory, we analysed the penile skin on the right side of penis. We could see an island of hairless skin which was possibly remnant of foreskin (prepuce). We designed a long flap from this with very good blood supply from the underlying dartos tissue. This flap was used in an onlay fashion to repair hypospadias in a single stage. Finally head of the penis (glans) was also repaired and a catheter was placed to drain urine till the whole repair healed. Rajveer went back to his home town in Haryana after 3 days of surgery and then we arranged for one of my surgeon colleagues in Hissar (Dr Vivek Gupta) to help with post hypospadias surgery care. The catheter was removed on day 14 since it was a major hypospadias reconstruction. Rajveer passed urine well and but the full healing of the penis took about 4 weeks. The final cosmetic result was excellent when he sent the pictures via email. He visited us recently almost 3 months after surgery, the penis looked well healed and he is passing urine well from the tip in a good stream. His erections are straight and he is now looking forward to get married. The only question is whether he will invite me for his wedding or not and if he invites, how will he introduce me.</p>
<p>When such complex failed cases do well after hypospadias treatment at Hypospadias foundation, it makes us very happy. We feel our goal of starting the foundation in Nov 2008 has been fulfilled. Last six years have been a long and exciting journey but is has been a very challenging time. When we started we never knew the burden of problem was so high.  Along the way we have been privileged to be a part of lives of more than 600 kids and adults with Hypospadias from all over India and some countries abroad. Almost 25% of those receiving treatment at Hypospadias Foundation, have been patients with failed hypospadias who received surgery elsewhere. Though initially we started only with management of hypospadias in children but we realized that there is huge gap when it comes to treatment of hypospadias in adults. Along with Dr Manish Dubey, my close friend who is an adult urologist we are able to offer good care and success rates in adults and failed hypospadias.</p>
<p>In last three years, Hypospadias Foundation has welcomed many patients from many other states and countries such as Nepal, Bangladesh, Greece, Afghanistan, Iraq, Saudi Arabia, UAE-Dubai, Oman, Great Britain (UK), USA, Nigeria, Tanzania, Kenya, Congo, Zambia and this number continues to grow attesting to our devotion to the field of Hypospadias. 			<p>Hypospadias surgery is a delicate, skillful and complicated surgery. The good thing about hypospadias treatment is that young babies heal very fast and become well within a week to ten days after the hypospadias repair. Fortunately, unlike adults, the children do not think about pain. They live their life happily soaking in, everything moment to moment. Adults on the other hand think and anticipate pain too much hence they land up having more pain. It is the anticipation of the pain which is worse than the pain itself.</p>
<p>That being said, here we have tried to give detailed post hypospadias repair instructions for the parents and patients so that they can understand the care which needs to be taken after hypospadias surgery. Please note that these are generalized instructions after hypospadias surgery and there will be some variations on a case to case basis and it is best to discuss with your hypospadias surgeon for other details.</p>
<ul>
<li><b>Diet:</b> Children can often be fed 2-3 hours after hypospadias repair if the surgery was done under sedation and caudal epidural anesthesia. Some complex or failed hypospadias may require general anesthesia for surgery and these kids are kept empty stomach for 3-4 hours after surgery.</li>
<li><b>Urine passage</b>: In almost all the hypospadias repairs, there is urinary pipe called catheter which is left across the hypospadias repair site. This goes into the bladder and drains drops of urine continuously. It is generally removed after 5-10 days by the hypospadias specialist depending on the type of the repair.</li>
<li><b>Diaper care</b>: Post hypospadias treatment babies are kept in double diapers till the time the catheter is there. If the child is more than 6-7 years of age, then diapers may be difficult to maintain- hence we prefer to use a urine bag for drainage of urine. We ask the parents to make a hole on the front side of inner diaper and bring out the catheter to drain into the outer diaper. This way the inner diaper is changed only when the child passes motions and hence remains dry. Outside diaper is changed whenever it is full of urine.</li>
<li><b>Hypospadias Dressing care:</b> At the end of hypospadias surgery, we place a soft gauze roll dressing on penis to give some support to the healing penis. There may be minor bleeding into the diaper on and off for first 3-4 days which is nothing to worry about. The hypospadias dressing sometimes becomes loose and comes out. If this happens within first 2-3 days, then we change the hypospadias dressing. If it happens later, we just remove the dressing and leave the wound open.</li>
<li><b>Discharge after hypospadias surgery</b>: Most of the children who have a simple primary hypospadias are discharged the same day of surgery by evening- daycare hypospadias surgery. Children who are traveling from far may come the night before to hospital and stay for a day or two after surgery as well. Children who have undergone a failed hypospadias or a complex surgery are kept in hospital for 2 nights after surgery also for antibiotic injections or pain relief etc.</li>
<li><b>Pain relief</b>: We prescribe an oral syrup of analgesic medicine mostly a combination of ibuprofen and paracetamol for first few days after surgery. This ideally should be given post feeds and not on empty stomach. We also give a medicine called oxybutynin which is an anticholinergic and prevents bladder spasms (painful bladder contractions due to catheter).</li>
<li><b>Medications:</b> A broad spectrum antibiotic such as cephalexin is also prescribed to prevent infections along with an antacid medicine such as Lansoprozole for the first few days.</li>
<li><b>Activity, walking, playing</b> within the house is allowed even the day of surgery by evening but we ask the parents to make sure that the child does not use cycle or any toy on which straddling is needed. Child can sleep on his tummy if he likes to sleep that way. Double diapers prevent too much compression of the hypospadias repair site.</li>
<li><b>Visits with doctor</b>: We like to see the patients on day 5 after surgery for removal of dressing and for some minor hypospadias repairs we remove the catheter also at the same time. If its moderate or severe hypospadias surgery, we generally remove the catheter after 7-10 days and hence the catheter is removed on second visit then.</li>
<li><b>Bathing</b>: Once the dressing is out on day 5, the child can be given a short warm tub bath. We ask parents not to use any cotton or cloth for drying the area of hypospadias repair surgery as fibers can stick to the raw area. It is better than after drying the rest of the body the diaper is put on directly and that will dry the area.</li>
<li><b>Ointments on penis</b>: We advise putting a broad spectrum antibiotic ointment such as neosporin locally on the penis at each diaper change so that it prevents infection and also protects the raw area from sticking to the diaper.</li>
</ul>

			<p>Hypospadias is one of the most common malformations in boys and affects more than 1 lakh kids in India. At Hypospadias foundation and MITR Hospital, Kharghar, Navi Mumbai, India – we see and manage more than 200 kids every year who have hypospadias and need surgery. These cases are either primary (75%) or those have failed surgery elsewhere (25%)- some of them have failed multiple surgeries elsewhere.</p>
<p>We strongly believe that hypospadias surgical correction should not be taken lightly or casually by any surgeons as first hypospadias surgery is the best chance which the child has. Once the first hypospadias surgery has a complication, more surgeries are needed and still the result may not be optimal. Hence, the surgeon needs to be well trained for hypospadias repair procedure and should have a very keen and passionate interest before he takes up any child up for surgery for hypospadias.</p>
<p>Amongst the various techniques for hypospadias surgery, one of the most popular one is Tubularised Incised Plate (TIP) urethroplasty. TIP repair was first described by Prof Warren Snodgrass and it currently constitutes 70% of all distal and midpenile hypospadias repairs across the world. Due to its universal adoption and remarkable results, it is also known as Snodgrass repair. I had the good fortune of assisting Dr Warren Snodgrass during his multiple visits to India when I was a resident in pediatric surgery at All India Institute of Medical Sciences. I keep bumping him into now and then at pediatric urology conferences, the last one of which was European Society of Pediatric urology Congress in Genoa Italy. We received the best paper award at the conference and it was nice to see Dr Warren Snodgrass applauding us.</p>
<p>While TIP repair or Snodgrass urethroplasty has excellent results, case selection before surgery is very important. The criteria are:</p>
<ol>
<li>Distal or midpenile hypospadias</li>
<li>Mild to moderate chordee which is correctable</li>
<li>Wide pink elastic urethral plate.</li>
</ol>
<p>In certain selected proximal hypospadias, Snodgrass repair can be used but on a personal level- a long Snodgrass repair may not be the best option in most cases. We have started using Onlay Island flap repair in most of our proximal hypospadias and all distal/ mid penile hypospadias which are unsuitable for TIP Urethroplasty.</p>
<p>Steps of surgery:</p>
<ol>
<li>Degloving the penis and chordee correction – straightening of the curvature of the penis</li>
<li>Midline urethral plate incision</li>
<li>Neo-urethra formation on a catheter</li>
<li>Second layer coverage with either a dartos flap or spongiosum</li>
<li>Penile skin cover with Byars flap which helps in creating a midline closure on underside of penis. After the healing is finished, penis looks like just a circumcised penis. In some selected cases Prepucial reconstruction can be offered.</li>
</ol>

			<p>In my experience as an expert hypospadias surgeon, while doing TIP urethroplasty everything has to be done meticulously to prevent complications. If I have to narrow down on two things which have really lessened the complications of hypospadias repair with Snodgrass urethroplasty at our centre are:</p>
<ul>
<li>Fashioning a wide caliber neo-meatus- so that there is no narrowing later on</li>
<li>Providing a second layer cover on new urethra with either spongiosum or a dartos flap.</li>
</ul>
<p>Master Soham’s parents stay in Latur, Maharashtra and their son was diagnosed to have a distal penile hypospadias at birth. Worried about multiple surgeries and poor outcomes for hypospadias repair- They searched for the best hypospadias surgeon on the internet and landed up in our pediatric urology clinic at MITR Hospital in Kharghar, Navi Mumbai. We did the surgery using a Snodgrass repair technique in First week of May 2014. The surgery went well and child was discharged the evening of surgery. The dressing and catheter were removed 5 days after surgery and now one month after followup the child has an excellent outcome after hypospadias repair surgery. The cosmetic appearance is excellent with a meatus at the tip of penis, straight penis and just a circumcised appearance.</p>

			<p>Hypospadias repair surgery is a very delicate and demanding surgery. It tests the ingenuity of the hypospadias surgeon, surgery skills and most importantly experience. We have realized that hypospadias repair procedure is not a surgery which can be done casually or as one of the many surgeries which a surgeon does. Best results of hypospadias surgeries are seen only when the surgeon dedicates his time and energy in pursuing the art and science of hypospadiology. Results keep on improving day by day and year by year. With more than 600 hypospadias repairs done over last 5 years, Hypospadias Foundation at MITR Hospital, Navi Mumbai, India provides the best results in hypospadias surgeries in south East asia. Children have travelled from all over India and other countries such as Nigeria, Greece, Bangladesh, Iraq, UAE to get treated under Dr Singal’s care for hypospadias.</p>
<p>To get the best outcome in hypospadias, it requires a big team effort which includes hypospadias surgeon, assistants, well trained OT staff and post-surgery caring staff.</p>
<p>No surgery is free of complication and complications happen even in the best of hypospadias surgeon’s hands though they keep in decreasing with advancing experience. Whenever we see any child with hypospadias in our clinic, we make sure that we discuss in detail about the possibility of complications and the post hypospadias surgery outcomes. Some of these we discuss it further here:</p>
<p>Immediate complications after hypospadias repair:</p>
<ol>
<li>Bleeding – Penis is a very vascular organ with a lot of blood supply. Since hypospadias surgery involves lot of dissection of penis and making of various flaps, suturing them back in proper way is very vital in preventing this complication. The bleeding is usually minor and stops in 2-3 days. Earlier we used to apply heavy and tight dressings after hypospadias surgery but now we have realized that these actually delay healing as they compress the healing tissue. So presently the dressings which we use are very soft and light.</li>
<li>Infection- After any surgery, the immunity of the body goes down plus there is raw area which invites growth of infection causing bacteria.  Fortunately, when we do hypospadias repair in children, infection is very rare and even if it occurs is usually superficial and resolves with antibiotics. Various preventive actions are taken before hypospadias repair such as very sterile techniques in operation theatre, broad spectrum antibiotic at start of surgery and 8 hours after surgery, oral antibiotics after surgery and careful handling post-surgery.</li>
</ol>
<p>Short term complications after hypospadias repair:</p>
<ol>
<li>Urethral Fistula: Fistula implies leakage of urine somewhere from the newly formed urethral tube below the tip of penis. This occurs because of improper healing resulting from poor blood supply, infection, tight stitches or just poor surgical technique. Sometimes the urethral fistula after hypospadias surgery may heal by itself but mostly it requires surgery which is recommended only after 6 months of first hypospadias surgery. Fistula occurrence depends on experience of the hypospadias surgeon, severity of hypospadias and technique.</li>
<li>Meatal stenosis: This refers to a tight urinary opening after surgery of hypospadias. This can be seen even 2 weeks after surgery and may need regular calibration or minor cut back procedure called meatotomy in some cases.</li>
<li>Diverticulum: This refers to formation of a baggy distended urethral tube and is visible during passage of urine. This may be seen after an onlay flap repair for hypospadias as the skin flap expands and balloons during passage of urine. With careful trimming of the flap during surgery, this has become much less common and occurs in less than 1% of cases after hypospadias surgery.</li>
<li>Stricture: Sometimes during the healing phase, the new urethral tube may become narrow and cause obstruction to urine flow. This is noticed by poor stream, straining (application of force) during passage of urine and long time taken to pass urine. This may require further treatment in form of dilatation or a second surgery some months later.</li>
<li>Dehiscence/ breakdown: Very rarely, the whole repair may breakdown due to poor healing, infection or loss of blood supply requiring more surgeries. This is very uncommon and unfortunate event necessitating a complete redo repair of the hypospadias.</li>
</ol>
<p>With the newer techniques of hypospadias repair and growing experience of hypospadias surgeons, the complications have become much less common after hypospadias surgery and even when they occur they can be managed well. It is well said that “the prepared mind never knows much trouble”. Once we know that complications can occur in any surgery and be prepared for them, it is much easier to initiate preventive action during surgery itself. Also, it is vital to identify complications earlier so that they can be managed well in time before any long term damage occurs.</p>
